Age Eligibility for AccuCirc® Circumcision
Core policy: SafeCirc’s newborn male circumcision service is intended for infants 0–28 days of age. For infants born prematurely, corrected age may be considered, but prematurity does not automatically extend eligibility. Final eligibility requires clinician review and confirmation that the infant is medically, anatomically, and procedurally appropriate.
Policy
SafeCirc’s newborn circumcision pathway is intended for infants who are 0–28 days of age and who are clinically stable, healthy, and appropriate for an office-based procedure.
Infants born at term
For infants born at or after 37 weeks’ gestation, eligibility is based on chronological age. The procedure must ordinarily be completed by 28 days after birth.
Infants born prematurely
For infants born before 37 weeks’ gestation, SafeCirc may use corrected age when determining whether the infant remains within the newborn service pathway. Corrected age accounts for the number of weeks the infant was born before 40 weeks’ gestation.
A premature infant may remain eligible until 28 days corrected age, provided that the infant:
has reached an appropriate level of postmenstrual maturity;
is medically stable and growing appropriately;
has no unresolved respiratory, feeding, temperature-control, hematologic, infectious, or other neonatal concerns;
meets SafeCirc’s weight, anatomic, and procedural criteria; and
is approved by the treating SafeCirc clinician.
Prematurity does not automatically extend eligibility. Final eligibility is determined after review of gestational age at birth, birth and NICU history, current health status, growth, weight, penile anatomy, and physical examination.
Clinical review and referral
Infants with ongoing oxygen or respiratory support, feeding or temperature instability, significant anemia, unresolved neonatal illness, a known or suspected bleeding disorder, abnormal penile anatomy, ongoing infectious concern or other clinical concerns require individualized physician review. The procedure may be deferred or the infant may be referred to pediatric urology setting when appropriate.
